ONLINE FORM REQUIREMENTS!  Every athlete and their parent/guardian must have a free Planet HS account. If you created an account previously, you just need to log back in, update information and upload your student's new MHSAA sports physical. Sports physicals on paper will NOT be accepted in the office.

A current MHSAA Physical is required for participation. The MHSAA form must be signed by a doctor after April 15th, 2023. Forms are available in the office or are linked at the side on this page.
